Would combining fire with multiple characters grant extra +4 attack? +4,8,12,16,20,....?

Correct, as long as the unactivated non-melee characters are in LoS to an enemy, only then you can choose who to combine fire with (to that same enemy within LoS). Also, combine fire doesn't violate stacking.
