Virulent Paralyzing Poison [+20 Damage against adjacent living enemies and target is considered activated this round; save 16]
Does the activation effect require a successful hit, or is the Save 16 triggered simply by being targeted?
In other words, if the attack misses, does the target still have to make the Save 16 and risk being considered activated for the round?

I'm pretty sure it has to hit to trigger.

The target only rolls the save if the attack hits. Both effects (not just the damage) only apply to adjacent living enemies, and both are avoided if the save is made.
